Survey Verification Instructions
2004 Civil Rights Data Collection

Thank you for participating in the 2004 Civil Rights Data Collection Survey. If you have not submitted your data or received notification that your survey is not complete, please follow these instructions:

  1. Log on to the web-survey tool.
  2. Print an Individual Survey Report-This report provides the status for each school/program in the district:
    • On the Survey Home page-Click <ED102> in the first field and <View Reports> in the second field.
    • Click on <Individual Surveys Report>. On the Report Filter page-leave the Status at <Select> and the School at <All>. Click the SUBMIT button.
    • On the Download Report page-Click <Individual Surveys Report>. This will create a Report of all the schools in your district, including the status.
    • You want to see that the Status is <COMPLETE> and the Next Approval Level is <2>. If the Status and Next Approval Level are different, please access the school/program survey in questions to address any errors.
  3. Check to make sure all new schools/facilities/programs are added to your school listing.
  4. Check to make sure all closed schools/facilities/programs are deleted from your school listing.
  5. Address errors or enter additional data in each district/school/program survey.
  6. Submit all ED102s and the ED101.
  7. Check for errors and, if necessary, print another Individual Survey Report.
  8. When your survey is complete, print a copy of the Survey Report for your records (Print/Save Copy on the gray toolbar in each survey).
