2. Project Overview (5%) - One Page Maximum.
Provide a one-page overview describing the project. List the name, phone and address of the contact person. Should the described project be funded, this will be furnished to the press and state and federal agencies.
3. Continuous Improvement of District Improvement Plan (20%) - Two Page Maximum.
Specifically describe the steps that will be taken to implement the existing District Improvement Plan. What performance criteria will be developed to measure the effectiveness of the steps taken? How will a review of program effectiveness be conducted? How will that review lead to refinements in the District Improvement Plan? Who will be involved in this process?
4. Action Research (25%) - Two Page Maximum.
Describe the action research that will be undertaken. How will the existence of the problem and the results of the intervention be documented, analyzed and presented both locally and regionally by the participating researchers? As a result of the project and analysis, how will specific recommendations be made for use in refining the District Improvement Plan?
5. High Standards for All Students (20%) - Two Page Maximum.
How will this project improve student learning and achievement? How is the plan linked to high content and performance standards? How will teachers, parents and community members be involved in the goal selection process? What activities will be implemented to assist all students in achieving the standards? What are the criteria developed to measure progress toward the goals? How will student achievement be measured? Who will be involved in the evaluation of progress toward meeting the goals? How will this review lead to refinements in the District Improvement Plan?
6. Preservice and Professional Development Activities (20%) - Two Page Maximum.
How will preservice training and professional development activities lead to all students' attainment of the content and performance standards? How will these activities equip educators with the knowledge and skills necessary to participate in and lead continuous education improvement activities? How does your professional development plan coordinate with other staff development funds you may have received, such as Eisenhower Title IIB or Title VI funds? Who will be involved in the design and evaluation of the professional development activities? How will this review lead to refinements in the District Improvement Plan?
7. Budget and Cost Effectiveness (10%) - One Page Maximum.
How will Oregon Goals 2000 Subgrant monies be used on school improvement plan development activities, implementation of high content and performance standards, action research, preservice training and professional development? Describe the criteria developed to identify special needs schools. Identify the schools so designated and the criteria by which that school was designated special needs. No more than 5 percent of the total subgrant may be used by the fiscal agent for administrative purposes. The total amount requested may not exceed $5,000 per participating district plus $16 per student at the participating sites (calculated via ADM, March 30, 1995).

At least 50 percent of the funds awarded to the local district must be made available to individual schools with a special need for such assistance as indicated by a high number or percentage of students from low income families, low student achievement or other similar criteria developed by the local educational agency.

No more than 5 percent of the total subgrant may be used by the fiscal agent for administrative purposes.
9. Collaborative Agreement and Letters of Commitment(5%) - Two Page Maximum.
Describe the relationship between participating schools, districts, ESDs, parents, community business, institutions of higher education, and other new or existing partners. What are the roles and responsibilities of each partner? How does this collaboration specifically support the implementation of the district improvement plan action research project, and the proposed preservice training and development activities?
a. Note that the school 21st Century Schools Council chair for each school and the superintendent of each district must sign the cover page and the Statement of Assurances. The chair of the school board of each participating district must sign the cover page, Statement of Assurances and the School Board Approval page of this application.
b. Consortia of schools and/or districts must establish a broad-based panel to oversee development and implementation of the subgrant activities. The panel must include representatives of each school 21st Century Schools Council and the teachers, higher education partners and others who are participants in the action research project (see page 1). Include a list of panel members, indicating name, position and organization represented.
c. Include a list of all higher education partners and others collaborating in the project. ATTACH LETTERS OF COMMITMENT.
For each district participating in the Oregon Goals 2000 Subgrant project, attach ONE copy of the District's current improvement plan. The District Improvement Plan is the January 1, 1995, Certificate of Initial Mastery Plan as revised. It must include the eight-item narrative description and timeline.
Note: Do not attach any additional support materials, tapes, videos, etc. They can not be considered in evaluating the merits of your proposal.

Statement of Assurances
This is to certify that the Oregon Goals 2000 Subgrant project in the ___________________ School District meets the following conditions:
The site/consortia application has been approved by the school board. The approved project must be consistent with existing district policies, rules and contracts bargained under ORS 243.650 to 243.782.
As required under Goals 2000: Educate America Act, Sec. 309 (a) (2) (A), the district will implement their local district improvement plan. The Certificate of Initial Mastery Plan required under ORS 329.465 is the local district improvement plan.
None of the monies received through the Goals 2000: Educate America Act, Section 309 (a) and (b) shall be used to replace expenditures for existing programs that are a responsibility of the school district.
In accordance with Goals 2000: Educate America Act, Sec. 309 (a) (6) (B), at least 50 percent of the funds awarded to the local school district must be made available to individual schools with a larger number of disadvantaged students than the statewide average. The need may be based on factors such as a high percentage of students from low-income families, high percentage of students with limited English proficiency, low student achievement, or other similar criteria developed by the district.
In accordance with Goals 2000: Educate America Act, Sec. 309 (a) (6) (C), the local school district serving as the fiscal agent, may use up to five percent of the subgrant funds received in each fiscal year for administrative expenses.
By September 30, 1996, the district will submit a midterm progress report to the Oregon Department of Education. Within 30 days after the completion of the project on June 30, 1997, the district will submit a final evaluation report to the Oregon Department of Education.
In April 1997, participating school site(s) will present their programs and results at a conference organized by the Oregon Department of Education.
